NOTE: Insurance is not accepted unless your company offers you preauthorized “out of network benefits.” You will most likely need to pay out of pocket.  You will received a detailed receipt which you may submit to your insurance company for any reimbursement they offer.  Or you can saved your receipts and write the service off under “Medial” on your taxes.
